The order of work is what keeps a bathroom on schedule. Everything comes out first, including old boxing and tiling, so the walls and floor can be seen. Then first fix: hot and cold runs, waste pipes with the right falls, the shower feed, and any electrical first fix by an electrician. Then the surfaces — boarding, tanking behind the shower, tiling. Only then does the sanitaryware go in, and finally the sealing, commissioning and testing.
The single biggest cost driver is whether things move. Keeping the WC, basin and bath in their existing positions means connecting to what is there. Moving them means new waste runs at the right fall, which under a timber floor means lifting boards and sometimes notching joists to a permissible depth, and on a solid floor means chasing or building up. Moving a WC is the most constrained of the three because it needs the most fall and the shortest run to the stack.

Skipping it saves very little and is invisible until the ceiling below stains.
Plan the ventilation as part of the job rather than an afterthought. Building Regulations require mechanical extract in a bathroom, and a properly sized fan ducted to outside — not into a loft void — is what keeps mould off the new grout. It is an electrician's connection, but it belongs on the plan from day one.
Expect the plumber to be honest about what needs other trades. Tiling, plastering, electrics and flooring are each their own craft. A plumber who claims all of them may well be capable of all of them, but it is a fair question to ask, and the answer should be specific.

How long will we be without a bathroom?
For a full refit, expect the room to be out of use for most of the job — commonly one to two weeks depending on size and how much moves. Where a house has only one bathroom, the WC can often be reinstated temporarily overnight; say so at the quote stage so it can be planned in.
Does moving the toilet cost much more?
It is the most constrained of the three fittings because it needs generous fall over a short run to the stack. A short move is often straightforward; a longer one may need the soil pipe re-routing or a macerator. It is worth asking before the layout is finalised.
