The tell-tale is a faint trickle or a ripple on the surface of the water in the pan when nothing has been flushed. It is easy to miss, because modern cisterns overflow internally rather than out through a pipe on the wall — so unlike the old design, a failed valve makes no mess and no noise outside. On a water meter, it is often the entire explanation for a bill that has doubled.
There are two failure points. The flush valve — the outlet mechanism in the middle of the cistern — seals against a rubber washer that hardens and distorts over time, letting water seep past into the pan continuously. The fill valve on the inlet side fails differently: the diaphragm wears, it stops shutting off cleanly, the level creeps up and water runs over the internal overflow into the pan. Which one it is, is determined in about a minute with a strip of dry paper against the back of the pan.
A simple test anyone can do: put a few drops of food colouring in the cistern, do not flush, and come back after half an hour. Colour in the pan means the flush valve is passing. No colour but a rising water level means the fill valve is the problem.
Repair is nearly always better value than replacement. The parts are standard — flush valve seals, diaphragms, whole valve assemblies — and a competent plumber carries the common ones. The exception is a concealed cistern behind a wall panel or in a furniture unit, where the access panel dictates what can be reached; those take longer, and a cistern built in without any access at all is a design decision somebody will regret.
Water use aside, the reason to fix it promptly is that a constantly wet flush valve seal will fail completely at some point, and a cistern that then cannot shut off at all is how a bathroom floor gets wet.
