Back boilers turn up regularly in this area's older housing, and they are usually still working, which is the awkward part of the conversation. They are robust, simple and very inefficient — typically well under 70% against a modern condensing boiler's 90%-plus — and parts for many models are no longer made. When one fails properly, replacement is the only route.
The removal itself is a job of moderate size. The fire front comes off, the boiler is disconnected from gas, water and flue and drawn out of the builder's opening, and the opening is then either made good and sealed with appropriate ventilation, or opened up for a new fire or stove. The existing chimney flue must be dealt with correctly — capped and ventilated, or lined for a new appliance — not simply boarded over, which traps moisture and causes damp.
Where the new boiler goes is the main design decision. A combi in the kitchen, a system boiler with a cylinder in an existing airing cupboard, or a wall-hung boiler upstairs. Each needs gas supply, flue route, condensate drainage and electrical supply, and each has different implications for the rest of the pipework.
The system almost always needs work at the same time. A house that has run on a back boiler has usually got an open-vented system with a loft tank, gravity or semi-gravity hot water, and decades of magnetite in the radiators. Converting to a sealed system, cleaning it thoroughly and fitting a magnetic filter is part of the job rather than an extra — a modern condensing boiler fed with that system's water will not last.
The upside is substantial: you get a chimney breast back, the efficiency change is dramatic, and the heating becomes controllable in a way a back boiler never was. The downside is that it is a proper job with a week's disruption, not an appliance swap.

Common questions
Can I keep the gas fire?
Sometimes a separate fire can be reinstated once the boiler is out, but it needs its own flue arrangement and a safety assessment. Many people take the chance to open the fireplace up or seal it properly instead.
Do I have to replace it if it still works?
No. But parts availability is the practical limit — when a component fails and cannot be sourced, the appliance is finished. Planning the replacement is better than being forced into it in February.
