What comes out is magnetite: black, magnetic iron oxide produced as steel radiators corrode internally. It settles in the bottom of radiators, chokes the narrow waterways of modern heat exchangers, and abrades pump bearings. It is the single most common reason a heating system that was fine ten years ago now heats unevenly and costs more to run.
The process matters as much as the machine. A pump unit is connected to the system, usually at the pump head or across a radiator's tails, and circulates water at high flow. Chemical cleaner is dosed and circulated hot to break the bond between the debris and the metal. Then each radiator is isolated and flushed individually in both directions, agitating the sludge in the base where it has settled — that individual, reversed flushing is what separates a real powerflush from an expensive circulation of dirty water. Finally the whole system is flushed to clear, neutralised, and dosed with inhibitor.
It is not right for every system. Very old pipework in poor condition can be disturbed by the process, and where corrosion has already perforated a radiator, flushing will find that weakness. A competent engineer assesses the system first and will sometimes recommend a gentler chemical cleanse instead, or advise that particular radiators should be replaced rather than flushed. That honesty is worth more than a guarantee.
A local point that gets misrepresented often: this is not a limescale service. United Utilities' data puts Macclesfield's supply between 45 and 68 mg/l depending on the reservoir zone, and scale only forms above around 200 mg/l. Anyone selling you a flush here 'for the limescale' is selling a solution to a problem the town does not have. The real enemy in soft water is corrosion, and corrosion debris is exactly what a flush removes.
Afterwards, the system should be protected so it does not return to the same state. That means a magnetic filter on the return to the boiler, catching magnetite continuously, and the correct dose of corrosion inhibitor with its concentration checked at each annual service. A flush without those two things is a temporary result.

How long does a powerflush take?
Most of a day for a typical house — commonly four to eight hours depending on the number of radiators and how dirty the system is. Anyone offering to do a whole house in an hour is circulating water, not flushing it.
Will it fix my cold radiators?
If they are cold at the bottom because of sludge, yes. If they are cold because of a seized valve, poor balancing or a failing pump, a flush will not help — which is why diagnosis comes first.
