Open-flued appliances are the ones where an annual check earns its money. Unlike a room-sealed boiler, an open-flued gas fire takes air from the room and discharges into a chimney, so a blocked or degraded flue, a failed chimney liner or an extractor fan pulling against it can bring combustion products back into the room. A flue flow and spillage test is the part of the service that checks exactly that.
The service itself covers the burner, the injector, the pilot and thermocouple, the radiants or fuel bed, and the oxygen depletion safety device that shuts the appliance down if the room's oxygen level falls. Balanced-flue and room-sealed gas fires solve the ventilation problem entirely by taking air from outside, which makes them a good fit for well-sealed modern rooms, at the cost of needing an external wall.
A carbon monoxide alarm belongs in any room with a gas fire. It is a legal requirement in rented property in England, and it is basic sense everywhere else — an open-flued appliance is the classic CO scenario, and the gas itself gives no warning.

Common questions
Why does the fuel bed layout matter?
Because the manufacturer's arrangement is what the appliance was tested and certified with. Moving coals or adding extra can obstruct the burner and produce carbon monoxide from a fire that is otherwise in good order.
